Contact Us Form Login for Library Patrons
- Navigate to the Contact Us form from the About Us megamenu or from the main footer.

- Access the Contact Us Form
To access the Contact Us form, you must login or receive a one-time passcode by entering a valid email address.
- Login
- Select on the log in link to display the login modal screen. Enter the Queens Library Card/ Username and Pin. Continue with the current login process. Upon a successful login, the Contact Us form will be displayed.
- Email Address
- Enter a valid email address to receive a one-time passcode to access the Contact Us form and select on the “Send One-Time Passcode” button.

- Once it is sent, a new page will appear to enter the passcode. You will only have 10 minutes to use the passcode.

- Check your email address for the passcode.
- Enter the passcode and select the “Submit Passcode” button. Once the patron is successfully verified, the Contact Us form will load.
- Follow the current process to fill out the Contact Us form.
- A successful Contact Form submission will display a message on the Homepage, “Help Request Complete. Thank you. We have received your information. Your help request number is INCxxxxxxxxxx. You should receive a confirmation email shortly.
